Strong Muscles, Strong Knees

Often, a muscle imbalance, in which one muscle or muscle group is stronger than another, causes knee problems. In other cases, lack of flexibility can contribute to knee pain or injury. In order for knees to function well, the muscles around them need to be both strong and flexible. You can do this by strengthening and stretching both the quadriceps (the muscles in the front of the thigh) and the hamstrings (the muscles at the back of the thigh). While rest is important when you injure your knee, too much rest can contribute to knee problems. Lack of use can cause muscles to weaken. Gentle exercise such as swimming can help keep muscles toned even when knees are a bit sore. And regular exercise can correct imbalances, increase flexibility, and prevent many injuries. However, not all exercises are healthy for the knees. Avoid loading the knee with weight when it's in a 90-degree position (that's the same angle it's in when you're sitting in a chair) or bent even more than that (such as in a baseball catcher's position), especially if you have kneecap pain. Unless you are getting up from sitting, avoid this position.


Causes of Hip Pain

These are some of the conditions that are most likely to cause hip pain:

  • Arthritis. Arthritic conditions such as osteoarthritis and rheumatoid arthritis are among the most common causes of hip pain, especially in older adults.  Arthritis leads to inflammation of the hip joint and the breakdown of the cartilage that normally cushions your hip bones. The pain gradually gets worse as the arthritis progresses. People with arthritis also feel stiffness and have reduced range of motion in the hip.
  • Hip fractures. Fractures of the hip are a particular problem in elderly people. With age, the bones can become weak and brittle. Weakened bones are more likely to fracture during a fall.
  • BursitisInflammation of the small, fluid-filled sacs (called bursae) that protect muscles and tendons is usually due to repetitive activities that overwork or irritate the hip joint.
  • Tendinitis. Tendons are the thick bands of tissue that attach bones to muscles. Tendinitis is inflammation or irritation of the tendons. It's usually caused by repetitive stress from overuse.
  • Muscle or tendon strain. Repeated activities can put strain on the muscles, tendons, and ligaments that support the hips. When these structures become inflamed from overuse, they can cause pain and prevent the hip from functioning normally.

Employment of physical therapists is projected to grow 36 percent from 2012 to 2022

Employment of physical therapists is projected to grow 36 percent from 2012 to 2022, much faster than the average for all occupations.
Demand for physical therapy services will come from the aging baby boomers, who are staying more active later in life than their counterparts of previous generations. Older persons are more likely to experience heart attacks, strokes, and mobility-related injuries that require physical therapy for rehabilitation.
In addition, the incidence of patients with chronic conditions, such as diabetes and obesity, is growing. More physical therapists will be needed to help these patients maintain their mobility and manage the effects of chronic conditions.
Advances in medical technology have increased the use of outpatient surgery to treat a variety of injuries and illnesses. Medical and technological developments also are expected to permit a greater percentage of trauma victims and newborns with birth defects to survive, creating additional demand for rehabilitative care. Physical therapists will continue to play an important role in helping these patients recover more quickly from surgery.
Furthermore, the number of individuals who have access to physical therapy services may increase because of federal health insurance reform. Physical therapists will be needed to assist these patients with rehabilitation and treatment of any chronic conditions or injuries.

Sciatica Pain Relief

As many as 40% of people will get sciatica, or irritation of the sciatic nerve, at some point in their life. This nerve comes from either side of the lower spine and travels through the pelvis and buttocks. Then the nerve passes along the back of each upper leg before it divides at the knee into branches that go to the feet.
Anything that puts pressure on or irritates this nerve can cause pain that shoots down the back of one buttock or thigh. The sensation of pain can vary widely. Sciatica may feel like a mild ache; a sharp, burning sensation; or extreme discomfort. Sciatica can also cause feelings of numbness, weakness, and tingling. Sciatica pain may make it difficult to be active. But bed rest is not recommended as a mainstay treatment. 
 To manage new sciatica pain, you may find that certain positions and activities are more comfortable than others.If symptoms are not severe but persist beyond a couple of weeks, your doctor may recommend physical therapy. The proper exercises may actually help reduce sciatic pain. They can also provide conditioning to help prevent the pain from coming back. The exercises recommended will depend on what's causing the sciatica. It's important to work with a specialist who has experience working with people with sciatica. Frank Klene, DPT, OCS, CSCS
Dr. Frank Klene is a licensed physical therapist practicing in the Indianapolis metro area. In addition to his Doctorate in Physical Therapy, Dr. Klene is a Board Certified Specialist in Orthopaedic Physical Therapy as well as a Certified Strength and Conditioning Specialist. He has co-authored several research studies, including a publication in the Journal of Manual and Manipulative Therapy.

While Dr. Klene treats a variety of patients and diagnosis, he holds a special interest in spine and sport rehabilitation, as he has personally battled with pain throughout his adult life requiring two spinal surgeries.
